The Hounslow Public Health Team are inviting colleagues to book onto the new Population Health Management Framework (PHMF) Lunch and Learn events to help better understand how PHMF can support health and care projects.
The first virtual Lunch and Learn for 2024 is on Wednesday 24 January, 1pm – 2pm. The session will focus on how best to use data and segmentation tools to better understand local health problems, and how to develop proactive interventions.
The Data and Segmentation Lunch and Learn event will include:
- Examples from colleagues across the Hounslow Borough Based Partnership (BBP) on how they have implemented these tools within their projects.
- Resource guides and available tools to understand the local population, including the Data Hub and WSIC.
- Recent case studies examples where teams have segmented data to understand inequalities and develop interventions.
- Current project examples linked to residents living in the CORE20 (most deprived) areas of Hounslow.

The Research and Engagement training elements of the PHMF have been divided into two virtual Lunch and Learn events:
- PHM: Research and Engagement Part 1 - Thursday 22 February, 1pm - 2pm. Covering methods and approaches, and how to reach your desired segmented groups to help inform your projects objectives.
- PHM Research and Engagement Part 2 - Wednesday 13 March, 12pm - 1pm. Focusing on the best research tools to help gather insights when engaging with residents.
The Population Health Management Framework has supported the following projects:
- Cervical cancer screening engagement
- Covid-19 vaccination uptake
- Blood pressure at home monitoring programme
- The new Falls Prevention Service
- Chelsea and Westminster’s Establishing a Population Health Management Clinic in surgical pre-assessment unit at West Middlesex University Hospital.
PHMF helps to:
- Use data and engagement to understand the needs of our residents
- Identify and target those who will benefit most from interventions
- Develop evidence-based interventions
- Embed evaluation to understand impact.
The BBP is keen for more health and care colleagues to undertake the training so that more projects can benefit from this data led approach within their project planning or evaluation processes.
