The Hounslow Public Health Team are inviting colleagues to book onto the final Population Health Management Framework (PHMF) Lunch and Learn events to help better understand how PHMF can support health and care projects.
The Research and Engagement training elements of the PHMF have been divided into two virtual Lunch and Learn events:
- PHM: Research and Engagement Part 1 - Thursday 22 February, 1pm - 2pm. Covering methods and approaches, and how to reach your desired segmented groups to help inform your projects objectives.
- PHM Research and Engagement Part 2 - Wednesday 13 March, 12pm - 1pm. Focusing on the best research tools to help gather insights when engaging with residents.
The Population Health Management Framework has supported the following projects:
- Cervical cancer screening engagement
- Covid-19 vaccination uptake
- Blood pressure at home monitoring programme
- The new Falls Prevention Service
- Chelsea and Westminster’s Establishing a Population Health Management Clinic in surgical pre-assessment unit at West Middlesex University Hospital.
The BBP is keen for more health and care colleagues to undertake the training so that more projects can benefit from this data led approach within their project planning or evaluation processes. As this will help you to better understand where interventions can produce positive outcomes for residents and patients.
