We are excited to announce the launch of the new broader Enhanced Dementia Care Service (EDCS) in Hounslow.
The EDCS is a multi-organisation service that consists of health, social care and voluntary services who provide community-based support for people living with Dementia with their diagnosis to help prevent crisis, unwarranted hospital admissions, and enable Hounslow residents to live independently.
To gain a better understanding of the EDCS please watch the following quick video and hear first-hand from one of the boroughs carers about how the service has helped change their life.
The EDCS provides those living with Dementia:
- A named coordinator
- Dementia nurses
- Dementia Co-ordinators
- Social Work Assistants
- Nursing associates
- Occupational therapists
- Coming soon, community support seven-days a week.
Originating with funding from Social Finance, the new broader service has been funded by the Hounslow Borough Based Partnership (BBP) for two years and will be managed and delivered by Hounslow and Richmond Community Healthcare Trust in conjunction with Hestia.
The Launch event was hosted by Anne Stratton, Director of the Borough Based Partnership (BBP), Steven Forbes, Director of Children’s and Adult Services at Hounslow Council, Councillor Samia Chaudhary; Cabinet Member for Adults and Health Integration, Hounslow Council, Alzheimer’s Society, and members from the EDCS team Felice Fisher (Service Manager) and Luki Obro (Clinical Lead) at Hounslow House on Monday 17 April.
Anne Stratton, Director of The Borough Based Partnership (BBP), said:
“It was so great to see so many of our partners coming together for the Enhanced Dementia Care Service launch from the Council, Alzheimer’s Society, Hestia, HRCH and the BBP.
With the EDCS working together with our colleagues in Primary Care, Social Care, Mental Health Services, Hospitals, and the Voluntary Sector we are now able to help more people living with dementia to live independently and prevent unnecessary hospital admissions in Hounslow.”
