Purpose
The Health Outreach Team (HOT), funded by the NWL Inequalities Transformation fund, aims to change the way we engage with residents as a Borough Based Partnership.
The team use population health data to understand where our residents with the greatest need and inequalities are and go to them to have conversations, build trust and understand their experiences. They work with the BBP to help bring services into the community, either at the Hounslow Treaty Centre Shop or via the (HOT) community bus visiting various pockets of the borough.
- To reach residents impacted by health inequalities and increase referrals for preventable services.
- Increase awareness for health services amongst target groups
- Use insight gathered from engagement to inform service design, service improvements and communications.
- Increase awareness for risks associated to obesity and long-term health conditions.
From April-October 2023, the team engaged with over 14,000 residents. They also offer winter vaccinations, BMI and blood pressure readings to help raise awareness.
The team have engaged with a variety of communities including faith groups, traveller communities and asylum seekers.
They have also collaborated with services such as social prescribers, sexual health, and ARC drugs and alcohol services.
Their evaluation plan enables them to monitor who they are speaking to, about what, and when. They also record what residents are saying about a variety of topics (e.g., Diabetes, Mental Health) and the themes from these conversations will help inform a variety of BBP services.
