Case Study Background
Sitting within the broader Frailty Programme, Hounslow’s Intermediate Care Ambition is focused on improving the intermediate care offer in Hounslow so that there is good community step-up provision, preventing avoidable hospital admissions and residential care placements for over 65s.
It also aims to optimise pathways by focusing our expertise on patients to whom we can make a material difference to their wellbeing and rehabilitation. Intermediate Care is one of the BBP’s six key areas of focus from its wider, ambitious programme of work and has been identified as one of the most relevant and impactful to the needs of Hounslow’s population in terms of transforming service delivery.
To review the Intermediate Care offer from a staff perspective.
A series of staff engagement events delivered so staff could fully understand the service offer, build partner relationships, undertake critical analysis, be involved in peer review and collaborate on effective change.
Once staff engagement sessions were concluded a dedicated BBP Managers session was delivered by Liz Sargeant, Integration Health & Social Care Lead. The session was designed to tackle fundamental questions about the service and wider pathways in the system. It aimed to generate ideas and contributions on how to effect change and transformation across Intermediate Care services.
There were 40 representatives from Joint commissioning (older adults, extra care, preventative services) Public Health (Frailty and End of Life Care (ELOC)) Adult Social Care: LD & Autism, Mental Health, Occupational Therapy Integrated Community Response Service (ICRS), Community Recovery Service (CRS), District Nursing and Anticipatory Care, Community Neuro Rehab, Hospital Social Work team, Enhanced Dementia Care Service, Let’s Go Outside and Learn (VCSE), Social Prescribing, Community Solutions, Extra Care providers, Discharge Hub, West London Trust – older adults, West Middlesex Hospital, Therapies, Hounslow Council Falls Coordinator and the British Red Cross (VCSE).
Next Steps
There is work to be done to streamline services and optimise the Intermediate Care offer but we felt that the person-centred practice of this workshop helped us engage the right people in achieving our goals.
